Collecting Evidence for your SVQ

SVQs are awarded on the basis of work carried out during the course of your training. Your assessor will help you choose which pieces of evidence best reflect your progress. All of the evidence which you collect and present for assessment must be relevant to your SVQ.

Each SVQ candidate has their own assessor, who will be the person that will support the candidate to plan their assessment, help them work towards completing their qualification and assess the work which they have gathered. An Internal Verifier checks off each unit that the candidate completes.

SVQs are candidate-led and, with support from their assessor, all candidates will be encouraged to decide which unit they want to be assessed on.

Evidence Forms

Here are some of the SVQ Evidence forms which you will use during the course of your SVQ. You can download them to your computer or print them out.

Evidence Sheet is used to record the main body of your evidence including Reflective Accounts, Products and Questions which are completed by the candidate and Direct Observations/Professional Discussions which are completed by your assessor.

Witness Testimony is for those occasions when you assessor is unavailable to observe you, in such instances another person can comment about your performance.

Evidence Index is essential for your assessors and verifiers to find their way around your portfolio. Each piece of evidence you gather should have an individual log number. Your evidence is logged in numerical order. You should fill out a seperate evidence index sheet for each unit you complete.
