Lifestyle Management is an exciting new addition to the Community Connectors service, launching in 2019. The service will support older people living with one or more long term conditions to take positive steps towards better self management.

What is self management?
Self management is an outcome achieved when an individual is informed and empowered enough to take ownership of their long term conditions.
The Lifestyle Management service takes a person centred, solution focused approach to self management. We work with the person living with the condition, not with the condition itself. We build on the individuals existing skills, strengths, resources, hopes and recovery strategies to help empower them to increase their control, coping and ability to move on with life.
A person who can self-manage should be able to make the best use of health resources, be at the centre of their own planning and feel confident that they have the tools to live the best life they can no matter what their diagnosis.

WHAT DO WE OFFER?
We offer 8 week, bespoke group courses facilitated by a Lifestyle Management Practitioner to allow older people to benefit from shared experience, with course content informed by the participants and input provided by condition specific, partner organisations.
One to One Surgeries
One-to-one surgeries are offered where immediate or more targeted interventions are needed to support a person towards better self management.
WHO IS THE SERVICE FOR?
The service is open to anyone over 55 across the city who is suffering from one or more long term conditions or facing any significant barriers to effective self management.
